What is Community Integrated Health?
The work of Community Integrated Health is driven by the understanding that 80% of health outcomes are due to factors such as health behaviors, social and economic circumstances, and the physical environment where people live.
People live their healthiest lives when they have access to nutritious food, affordable housing, health care, quality education, clean air, and safe communities. However, efforts to improve health have historically only focused on the traditional health care system rather than how community-level factors could help prevent illness and promote well-being. Building bridges between clinical and community approaches to care could lead to better health outcomes for all.
Community Integrated Health aims to strengthen the relationships between traditional health care systems and community-based organizations in order to help all community members live their healthiest lives.

OUR COMMUNITY INTEGRATED HEALTH PROGRAMS
ACTIVELY CHANGING TOGETHER! (ACT!)
ACT! is an evidence-based healthy-lifestyle program to help overweight youth and their families make sustainable health behavior change. The program aims to help youth and their guardians make changes in activity and eating that can benefit their health and well-being.
BLOOD PRESSURE SELF-MANAGEMENT
Our Blood Pressure Self-Monitoring Program is designed to help participants reduce and better manage their blood pressure, increase awareness of triggers that elevate blood pressure and enhance knowledge to develop healthier eating habits.
YMCA'S DIABETES PREVENTION PROGRAM
The YMCA’s Diabetes Prevention Program is a 12 month, small-group program that helps people who are at high risk for type 2 diabetes reduce their risk by making simple, healthier lifestyle choices. Risk for developing type 2 diabetes may be reduced or eliminated by weight loss, healthier eating and increased physical activity.
LIVESTRONG AT THE YMCA
LIVESTRONG at the YMCA is a free program tailored to meet the specific needs of adult cancer survivors who would like to improve or reclaim their health and well-being before, during and after treatment. By focusing on the whole person and not the disease, LIVESTRONG at the YMCA helps participants move beyond cancer in spirit, mind and body.
HEALTHY KIDS DAY
Our Healthy Kids Day is a FREE family friendly event with healthy interactive activities, inflatables, community vendors and a fun obstacle course that kids and adults can both enjoy. This is an official Kentucky Derby Festival event, and is held annually in April.
